care for your new puppy from Lakeshore Kennel.

You will need to read each section listed below, then answer the
appropriate questions provided. Please press the submit button at the
bottom - which will send us your information.  This will assure us that you
have all of the appropriate information you need to properly care for one
of our puppipes.

THIS MUST BE DONE PRIOR TO ANY PUPPIES BEING SHIPPED OR
PICKED UP AT OUR HOME.
